Driver’s License Reinstatement — DMV Drug & Alcohol Evaluations

Many states accept telehealth evaluations for license reinstatement, but policies can vary by state and sometimes by local office.

• Some DMVs accept telehealth evaluations outright; others require a provider from a state-maintained list or an in-state agency.
• For DUI-related suspensions, certain states require a local provider or prior permission.

Accepted examples: Our evaluations have been accepted by DMV in AL, AK, AZ, CA, CO, FL, IA, LA, MA, MN, NY, ND, OH, PA, SC, SD, TX, WV, WY; IL (out-of-state packet only).

Best practice: Ask your DMV or licensing authority if a telehealth evaluation by a licensed/certified provider is acceptable for your case, and whether a state-specific provider list applies.

• Photo ID and contact info.
• Reason for suspension (e.g., DUI, refusal, points) and any case paperwork.
• Court orders, probation terms, or DMV letters (if applicable).
• Any prior evaluations, education/treatment certificates, or discharge summaries.
• Your driving record (if available) and relevant test results.

Bringing complete documents helps avoid follow-ups and speeds delivery.

• Standard: Typical delivery in ~48 hours after your appointment when no extra steps are required.
• Expedited: Priority review with typical ~24-hour delivery when no extra steps are required.

Missing documents, required testing, or third-party verifications can extend timelines.

This can sometimes change from state to state.

• Where states allow you to hand in the report, we will provide it to you directly to submit.
• In states that mandate communication directly from the evaluator, we will send the form for you to the designated agency.

Tip: Some DMVs require uploads through their portal or delivery at a field office—follow your DMV’s instructions.

Requirements depend on your history and state rules. Your evaluator may recommend education, treatment, and/or lab testing as appropriate.

• Any classes/treatment/testing are billed by those providers and are not included in our fee.
• We can provide referrals and documentation of completion when needed.

If a specific in-state provider list is required, ask whether telehealth providers licensed/certified in their state are acceptable.
