Starting your drug and alcohol evaluation is simple and convenient—you can begin whenever you’re ready. The first step is to visit the Start Now page, where you can book an appointment and make a payment for services. Once you’ve booked the appointment and made the payment, you’ll immediately gain access to complete the online questionnaire, which should be completed before your appointment. Then, at your scheduled appointment, you’ll meet with the counselor to complete the evaluation process via video. Following the video session, you’ll receive the results via encrypted email.

Billing

Technical Difficulties

We use Zoom for our video telehealth sessions. Begin by testing your microphone and camera before joining the evaluation. Ensure to access the link emailed to you when you booked your appointment. Adjust your audio and video settings as necessary and situate yourself in a quiet, well-lit area for optimal visibility. Engage actively with the evaluator during the session, maintaining clear communication and eye contact with the camera. Should any technical issues occur, promptly notify the evaluator for assistance. Upon completion of the evaluation, end the Zoom meeting appropriately.

Upon successful payment, the questionnaire will load automatically. However, in case of a computer crash or if the window fails to load properly, please check your email inbox for a message from us sent to the email address you provided during the purchase. This email will contain a link to regain access to the questionnaire. If you do not receive this email or encounter difficulties locating it, please submit a support ticket or reach out to us directly at (888) 648-2012 for further assistance.

Post-Evaluation Support

To address errors in the report, submit a support ticket or reply to the email containing the report with specific revisions needed. Provide clear details and any supporting information, ensuring accuracy in the final document. Follow up to confirm corrections have been made satisfactorily.

If you haven’t received your evaluation report, please check your spam or junk folder first. The report is typically a PDF document titled with your first and last name. If you still can’t find it, please submit a support ticket or call us at (888) 648-2012 for assistance.

To request your report sent to another entity, such as the court or attorney, please fill out and submit our Consent to Release Information Form. This form is necessary to comply with federal law regarding confidential information release. Ensure you include the recipient’s details at the bottom of the form.
