The U.S. Department of Transportation (US DOT) sets safety rules for transportation workers in safety-sensitive roles—like CDL drivers, pilots, railroad workers, pipeline operators, transit employees, and mariners. Under DOT rules, drug & alcohol testing is required. If you fail or refuse a DOT test, you must complete the Return-to-Duty (RTD) process with a DOT-qualified Substance Abuse Professional (SAP). The SAP evaluates you, recommends any education/treatment, and clears you to return to duty once you complete all steps.

Court or legal matter? Documents you should provide
If your evaluation is related to a criminal offense, whether involving pending charges or a conviction, or a legal matter, we recommend providing documentation related to the legal matter. Relevant documents may include a citation or ticket, arrest report, court order, probation terms, charging documents, or other paperwork related to the offense.

After booking your appointment, you will need to complete a pre-appointment questionnaire. At the end of the questionnaire, you may upload these documents directly for review as part of your evaluation.

Confirm your agency allows telehealth. If they require an in-person treatment program to perform the evaluation, our service isn’t a fit.

Client responsibility: You are responsible for following the requesting party’s instructions before purchase, including confirming acceptance of telehealth and any requirements for evaluation type, format, or evaluator credentials. If those requirements are not met, our service may not be accepted and may not qualify for a refund under our policy.

Per our refund policy, we don’t refund if the evaluation is for: license reinstatement, divorce court, custody cases, child services, professional boards (e.g., nursing), boards of counselors/physicians, safety-sensitive matters, or immigration.
