U.S. Department of Transportation (DOT) Substance Abuse Professional (SAP) Return-to-Duty (RTD)

Work 1-on-1 with a DOT-qualified SAP to complete RTD steps and get back to work safely.

  • Cost: Starts at $539 one-time, or installments from $134.75 at checkout.
  • Time: The DOT requires (1) an initial SAP evaluation, (2) completion of recommendations, and (3) a follow-up SAP session. Then you must take and pass your RTD test before resuming safety-sensitive duties. Many clients complete the SAP steps in under 1 week, but timelines extend if more than classes are required.
  • After you pass RTD: DOT follow-up testing continues for at least 12 months and includes at least 6 follow-up substance tests, per your SAP’s plan.
Confidential • Nationwide telehealth (except Nebraska) • DOT-qualified SAPs
  • Two SAP sessions are required by the DOT: an initial evaluation and a follow-up evaluation after you complete the SAP’s recommendations.
  • RTD test: You must pass a DOT Return-to-Duty substance test before you can return to safety-sensitive duty.
  • Follow-up testing: After passing the RTD test, your SAP will prescribe follow-up testing that lasts at least 12 months and includes at least 6 tests. Frequency may be higher based on your plan.
  • What’s not included in our fee: Any costs for drug/alcohol testing after your SAP sessions are paid to separate testing providers. If you need third-party services to document testing compliance or to complete classes/treatment, those providers bill you directly.
  • Timelines: “Under 1 week” reflects many straightforward cases; timelines vary if additional education/treatment or testing is required by the SAP.
  • Installments: Offered by third-party providers at checkout; terms, fees, and approvals are set by those providers.

CDL drivers after a violation (positive or refusal)

Your Clearinghouse says:

You are prohibited from operating a CMV

Why this hurts: your license is frozen, income stops, and recruiters move on. Many drivers also worry about having their CDL downgraded after spending time, money, and effort to earn it. It’s frustrating, stressful, and unclear where to even start.

How we help:

  • Get immediate guidance to designate a DOT-qualified SAP so your Return-to-Duty process can begin right away.
  • Follow a clear roadmap: evaluation → complete recommendations → follow-up → RTD test.
  • Our DOT-qualified SAPs will manage your case fully, helping you complete the initial steps needed to return to work quickly.

5RTD TestGreen light

Return-to-Duty test (negative required)

Take the RTD substance test at a DOT-approved site. You may return to DOT safety-sensitive duty only after a negative RTD for illicit drugs.

After a negative: Employer may restore you to duty and begin the SAP-directed follow-up testing plan (Step 6).

6Follow-Up PlanWorking

Find My Evaluation (60s)

Why can’t anyone give me a straight answer about how long this takes?

Let’s be honest — this is the question everyone asks first. Our counselors answer this question every day, and we know you’ve probably been searching everywhere for a clear answer.

Here’s why no one can give you a simple number.

The DOT has strict federal rules that every Substance Abuse Professional (SAP) must follow. Until we complete your evaluation, it’s impossible — and unethical — to promise how long it will take to get you cleared.

If another provider tells you, “We can have you done in three days” before even meeting you — that’s a red flag. Those shortcuts can delay your case or even cause compliance problems later.

We don’t make promises we can’t keep. Instead, we move fast, follow the rules, and keep you informed every step of the way.

Typical Timeline

Most straightforward cases — with little or no substance-use or legal history — finish the required SAP steps in under one week.

After That

Once you’ve completed your recommendations and follow-up, you’ll take your Return-to-Duty test. A negative result lifts the “Prohibited” status.

Why We’re Different

We’re ethical, fast, and transparent. You’ll never get vague answers or false promises — just clear steps and honest expectations.

Our goal: to get you back to work quickly, without risking your compliance or credibility.

No guesswork. No gimmicks. Just the truth and a fast, compliant path forward.

Why the cost is what it is

We know this can feel like a big expense — especially if you’re between jobs. Many of our clients are in the same position. You’re not paying for a single appointment — you’re paying for a federally regulated process with detailed oversight, documentation, and accountability by a qualified professional.

What goes into your SAP evaluation cost

Your DOT-qualified SAP manages your case from your first session until you’re cleared to return to duty. Behind the scenes, your SAP handles:

  • Initial evaluation and required follow-up in accordance with DOT standards
  • Review of history, test results, and documentation to ensure complete compliance accuracy
  • Federally compliant reports and recommendations submitted properly and on time
  • Employer/TPA communication — even if you change jobs mid-process
    • If you’re unemployed, your SAP will prepare the required compliance letters and send them to as many prospective employers as needed until you secure a position.
    • If you switch employers while in a Follow-Up plan, your SAP will update each employer and coordinate your testing so compliance remains continuous.
  • Long-term maintenance of your DOT records for the legally required retention period

This isn’t just an evaluation — it’s full case management that protects your ability to work, reputation, and future employment.

DOT SAP — Frequently Asked Questions

What exactly is the DOT SAP process?

The SAP process is a federally required pathway to restore eligibility for safety-sensitive work after a violation. It includes:

  • Designate a DOT-qualified SAP in your FMCSA Clearinghouse (or other DOT agency).
  • Initial SAP evaluation (telehealth available in most states).
  • Complete recommendations (education/treatment as required).
  • Follow-up SAP evaluation confirming completion.
  • Return-to-Duty (RTD) test and Follow-Up Testing Plan (minimum 6 tests over 12+ months).
How fast can I get started?

Availability for appointments can vary from week to week, depending on demand and scheduling volume. However, most clients find that we offer the soonest available SAP appointments compared to other providers.

Many providers have outdated processes, require in-person appointments hours away, or simply don’t respond to inquiries. Others make it difficult to book, resulting in long delays that keep you from returning to work.

We’re different. In many cases, appointments can be scheduled within 1–2 days or within 1 week’s time, allowing you to start your Return-to-Duty process quickly and confidently.

What will my employer see?

Your employer receives only information that the Substance Abuse Professional (SAP) is authorized to release under federal DOT regulations. This includes:

  • Initial Evaluation Summary – the purpose for participation and recommendations.
  • Follow-Up Evaluation or Session – confirms completion of required steps and eligibility (or not yet eligible) to return to safety-sensitive duties.
  • Follow-Up Testing Plan/Schedule – details the number and frequency of follow-up tests required over a 12–60 month period.

Employers and prospective employers also view your Clearinghouse record, which will show your SAP progress/status. The full evaluation report and clinical notes are not shared unless you provide written consent.

Can my CDL be downgraded while I’m resolving a violation?

Many drivers worry about a potential CDL downgrade after investing time, training, and money to earn it. State actions vary, but starting your SAP process promptly is the best way to reduce downtime and protect your license. We prioritize fast scheduling so you can move forward quickly.

Do you only help CDL drivers?

No. While over 90% of our clients are CDL drivers under the FMCSA, we also assist FAA, FRA, FTA, PHMSA, and USCG employees, as well as non-CDL employees referred under company policy or after a DOT physical.

How much does it cost? Are testing or classes included?

Your SAP evaluation fee covers the required SAP evaluations and return-to-duty documentation. Education, treatment, and drug/alcohol testing are paid separately to third-party providers. We’ll point you to cost-effective, compliant options.

Is this service available in all states?

We offer telehealth appointments nationwide except Nebraska (NE). If you’re located in NE or prefer in-person, we can guide you to compliant options nearby.

What is the Follow-Up Testing Plan (FUP)?

The SAP establishes a Follow-Up Testing Plan requiring at least six unannounced tests over 12 months or more. Employers/TPAs oversee the scheduling, and compliance with testing is required to maintain eligibility for safety-sensitive duties.

Will you tell me exactly what to do next?

Yes. Our DOT-qualified SAPs provide step-by-step guidance—from designation to your Return-to-Duty test. We manage your case fully and help you complete the initial steps needed to return to work quickly.

Are you affiliated with the DOT or FMCSA?

No. DrugEval is an independent service. Our clinicians are DOT-qualified SAPs who follow federal guidelines, but we are not affiliated with or endorsed by the DOT, FMCSA, or any government agency.

Can I complete the SAP / Return-to-Duty process while unemployed?

Yes. You can complete the SAP evaluation and SAP-directed education/treatment while not employed. For the Return-to-Duty (RTD) and Follow-Up testing plan, you’ll need a program to administer and document tests—this is typically handled by a consortium/TPA until an employer assumes responsibility.

I refused a DOT test. Do I still have to participate in SAP / Return-to-Duty?

Yes. A refusal is treated as a test failure under DOT rules. You must complete the SAP process (initial evaluation, SAP-directed education/treatment, follow-up evaluation) and obtain a negative RTD test before you’re eligible to perform safety-sensitive functions. You’ll also be subject to an SAP-prescribed Follow-Up testing plan.

How do I complete a DOT SAP as an owner-operator?

Book with a DOT-qualified SAP, complete the initial evaluation, finish any SAP-directed education/treatment, then complete the follow-up evaluation. Because you don’t have an employer, enroll with a consortium/TPA to run your RTD and Follow-Up testing plan as written by the SAP. Keep records to show “No Open Violations” in the Clearinghouse.

Who pays for DOT SAP services?

Payment policies are not set by DOT. Costs are typically paid by the driver, the employer, or shared—depending on company policy, insurance, or collective bargaining agreements. Confirm expectations in writing (SAP services, education/treatment, RTD and Follow-Up testing fees).

Can I be rehired after a DOT violation?

Yes—once you complete the SAP process, produce a negative RTD test, and remain in compliance with your Follow-Up testing plan, you become eligible. Hiring is at an employer’s discretion, but your Clearinghouse should show “No Open Violations.”

What are the requirements for the Return-to-Duty and Follow-Up testing?

The SAP must first determine you are eligible for RTD. After a negative RTD test, you return to safety-sensitive work under a Follow-Up Testing Plan set by the SAP. Plans require at least 6 unannounced tests in the first 12 months, and may extend up to 60 months. Your employer or consortium/TPA administers the plan exactly as written, and missed tests are violations.

For owner-operators or drivers between jobs, use a consortium/TPA to schedule and document tests until an employer assumes responsibility.
